PRIMARY FUNCTION:A licensed professional competent registered nurse who assumes responsibility and accountability for patient care activities in the Hospital at Home program in accordance with the Christiana Care Nursing Professional Practice Model. PRINCIPAL DUTIES AND RESPONSIBILITIES: Performs and documents accurate clinical assessments of patients on admission and throughout hospital stay according to established standards.
