Maintains documentation and paper flow from opening of participant paper chart through discharge.Job DutiesReceives requests for information from patient medical record.Pulls charts from the file area for all departments in the clinic and refiles all charts returned to Medical Records Department.Files reports and hospital discharge summaries in the chart.Updates chart tracking system for all records leaving and returning to the department.Answers phones and assists callers as necessary.Creates new medical record folders and maintains order of files.Researches medical record number and chart status.Performs verification of patients having more than one medical record number.Scans reports and documents into the required system.May train other employees on data entry for back-up coverage, as necessary.Maintains confidentiality and security of all Patient Medical Records and Information.Work is typically performed in an office environment.
