The MDS Coordinator in collaboration with the Unit Manager and Administrative Assistants and under the direction of the Director of Nursing, initiates and completes the MDS for the residents assigned. This MDS accurately describes and reflects the residents' current status. The Coordinator clearly, concisely and effectively communicates this resident information to the other members of the health care team for use in care planning, therapy participation, etc. The MDS coordinator must have a thorough working knowledge of geriatric nursing, CMS Long Term Care Standards and excellent assessment skills.
DUTIES AND RESPONSIBILITIES:
Assesses residents on admission and at least quarterly, as well as when conditions change. Documents appropriately including Minimum Data Sets.
Participates in the Admission Conference as well as the Quarterly and Annual MDS Quarterly Conferences.
Receives referrals from Acute Care, TCU, and Community and completes clinical screening in collaboration with Long Term Care Navigator.
Recommends bed placement to Unit Manager for optimal placement considering physical DME needs and room logistical support.
Collaborates with Administrative Assistant for appropriate facility notification on email regarding patient/resident admission, transfer and discharge.
Collaborates with Administrative Assistant for DME orders, O2 Concentrators and other patient support equipment.
Ability to adequately assess and reassess pain.
Assesses the resident's condition and nursing needs; sets goals; prescribes appropriate nursing action, as needed, and updates the Care Plan as applicable.
Documents changes in resident's condition and nursing care on progress notes, ID Notes and plan of care.
Performs all aspects of resident care and resident interviews in an environment that optimizes resident safety and privacy.
Submits documentation in timely, clear, concise and pertinent manner.
Notifies physician of changes in resident's condition, required documentation and follows through until appropriate action is taken.
Notes and carries out physician's orders in a timely manner.
Gives a thorough report to oncoming shift and documents appropriately on calendar for upcoming needs, on the rare time it this is needed by this position.
Follows through, as needed, on information given by shift report, resident or family regarding resident concerns.
Implements plan of care for the resident based on assessments and goals as established by the interdisciplinary care team.
Participates in IDTR weekly at Benefis Senior Care Center.
Treats residents, their families, visitors and co-workers with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of residents and their families.
Demonstrates competence in technical and manual skills, according to the scope of practice for RNs.
Functions as a resident advocate by protecting the resident's rights.
Participates in Admission, Quarterly and Annual Care Conferences.

Graduate of an accredited school of nursing, BSN preferred.
Current licensure as a Registered Nurse in the State of Montana.
Current BCLS certification, Health Care Provider, and maintained per Benefis policy.
RN, minimal 3 years, with recent clinical nursing experience in long term care.
