JOB TITLE: Senior Clinical Consultant- Population Health

Serves in a leadership role to manage complex projects, facilitate change, guide the development of data elements/sources, assure data accuracy and support data/information dissemination,drive substantial improvements in quality and efficiency and share accountability with clinical and operational leaders in achieving transformational results at both an internal and external program level. Responsible for providing subject matter expertise based upon previous experience with workflow redesign, organizational structure, supporting informational systems, and leading practices to identify opportunities for improved performance, increased effectiveness, and cost reduction.
This senior team member will facilitate change and take shared accountability with operational and clinical staff across the care continuum to achieve quality and efficiency improvement.

EDUCATION AND/OR EXPERIENCE:
Master’s degree in Nursing, Public Health, Health Administration, or related field is required. Bachelor’s degree plus relevant experience may be substituted for the Master’s degree.
3 to 5 years’ experience working in direct patient care environment. Some post-acute care experience is highly desirable.
Minimum of 5 years project management/leadership experience with proven ability to develop effective programs and deliver effective solutions (or commensurate experience) required.
Staff supervisory experience preferred.
