Transitions LifeCare is seeking an experienced clinical leader motivated to apply clinical and administrative skills to chart new territory in health care and influence innovation in high tech and high touch care models. Reporting to the Chief Executive Officer, the VP of Clinical Services, “Chief Innovation Officer”, is responsible for maintaining high quality care within all programs and services, increasing access to care, improving efficiencies and creating value for stakeholders. The VP of Clinical Services is a strategic partner with the CEO, CMO, Executive Team and clinical leadership in driving innovation, clinical excellence, and community partnerships to fulfill the mission, vision and strategic goals of Transitions LifeCare.

Oversee the management and strategic direction of Hospice, the Hospice Home, Home Health, Home Care, and supportive care including social work, spiritual counseling, and grief counseling.
Work with the Chief Medical Officer to integrate medical and supportive care within Transitions PalliativeCare, Transitions Kids, and Transitions CareChoices in order to support growth, sustainability and positive outcomes.
Supervise clinical directors, including the Director of Hospice and Home Health, as well as the Director of the Hospice Home.
Conduct organizational assessments of clinical operations, including organizational structure, efficiencies and clinical outcomes.
Identity opportunities for new strategies, programs and services that support the TL mission and meet the unique needs of patients, families, referral sources and community partners.
Develop and maintain networks, collaborations, and partnerships that foster innovation, create value, and achieve greater impact.
Work with internal leaders and external customers to ensure that TL programs are clinically integrated within the circle of care and along the network continuum.
Identify and remove internal and external barriers to collaboration, care coordination and access to care.
Oversee development and approval of appropriate policies and procedures.
Participate in budget process with Sr. VP of Finance & Operations and the Executive Team.
Maintain and promote a culture of customer service, customer satisfaction, and person and family centered care.
Maintain and promote a culture of quality and compliance.
Participate in educational activities and presentations for diverse audiences.
Participate in leadership development, staff development and staff support activities.
Direct and participate in performance improvement activities.
