Provides physical therapy interventions to accommodate the individual needs of the patient in the home care setting. Demonstrates knowledge and compliance with the patient's established Plan of Care (POC), identifying any barriers to successful implementation. Instructs patients and caregivers in exercise program, specific care techniques, and use of equipment as approved by the supervising therapist. Evaluates the patient's response to the POC and progress towards goals on an ongoing basis, reviewing the treatment plan with the therapist at least monthly. Demonstrates appropriate problem-solving abilities. Provides supportive documentation to justify service extensions.
Demonstrates knowledge and competence in standard professional practice, working within the parameters of licensure to implement the POC as delegated by the supervising therapist. Demonstrates knowledge of applicable Agency policies and procedures, state and federal guidelines, and CHAP standards.
Communicates effectively; provides timely notice of availability, reports any identified problems, schedule changes, or other factors altering the established POC. Communicates patient progress toward goals to the therapist prior to anticipated discharge to determine appropriateness of discharge or possible need for continuation, ensuring optimal patient outcomes.
Documents all coordination of care efforts and/or communication. Completes and submits paperwork in a timely manner in accordance with Agency requirements. Participates in person or by teleconference in clinical conferences, educational presentations, and performance improvement activities of the Agency; assists Agency in identifying educational needs and interests. Provides timely response to Quality Improvement requests for clarification and completion, ensuring correctness of documentation and accuracy of billing. Participates in weekend rotation as scheduled, Emergency/Disaster Plan as needed.
